Adult tetralogy of fallot compensated with aorto-pulmonary window

The patient who was operated on “pink tetralogy of Fallot” there were characteristic anomalies as well as a defect of 0.5x1 cmin diameter considered to be a type II aorto-pulmonary window. In acyanotic Fallot’s tetralogy with highpulmonary gradients, even if routine investigations do not provide additionl evidence suggesting pulmonary flow, this possibility should be considered. Otherwise, surprises are not avoidable during surgery.
