A rare localization of the bronchial rupture: middle lobe bronchus

Bronchial rupture generally results from blunt trauma. It is often observed at the right main bronchus, left main bronchus and distal portion of the trachea, respectively. The diagnosis is based on the clinical signs and fiberoptic bronchoscopic examination. The primary treatment is surgical repair. A 33-year-old male case was admitted to emergency department after vehicle accident with the complaints of dyspnea. Physical examination revealed multi-organ trauma and right tension pneumothorax. Tube torachostomy and closed under water drainage were performed. During the follow-up period, massive air leakage and bleeding continued. After bronchoscopic examination, we suspected bronchial rupture. Right thoracotomy and middle lobectomy were performed. The case has been asymptomatic for nine months. The case was presented, as middle lobe is a rare localization of the bronchial rupture.
