Single coronary artery is described as an isolated coronary artery originating from the aortic root through a coronary ostium without another ostium. Single coronary artery feeds the whole parts of myocardium. In this article, we report a 65-year-old male case admitted with typical chest pain who underwent surgery after coronary angiography and diagnosed with single coronary artery. Coronary angiography revealed a whole coronary arterial system originating from the right sinus valsalva and critical lesions in the proximal portions of both left anterior descending artery and right coronary artery. The patient was operated urgently and discharged in the postoperative fifth day.
