Internal mammary artery (IMA)has gained agreat acceptance as a conduit for coronary artery surgery during last years.

Arterial grafts are superior to venous grafts at midterm and longterm however some catastrophicresults may occur with arterial grafts but they are limited in amount.

IMA hipoperfusion is a catastrophic clinical syndrome which generally occurs at early postoperatve period or during weaning from cadiopulmonary bypass.

In this report we have presented a case with mammary artery hipoperfusion after cononary artery bypass surgery.
