CABG on the Beating Heart Without Using Pump Oxygenator in High Risk Patients

The first CABG surgery cases were performed on the beating heart, however because extracorporeal circulation and cross clamping the aorta provided optimal surgical comfort, this approach has gained wide acceptance worldwide. In high risk patients, however, this technique carries inherent risks that may results in limiting indications for surgery.

In our hospital we operated on 390 high risk patients with the beating heart technique. There was 3.07% early mortality and 12.8% morbidity. Results of postoperative echocardiographic, angiographic and clinical follow-up indicate successful surgical results. CABG on the beating heart is a safe and relliable alternate, without performing cardio-pulmonary pypass, in selected high risk coronary patients with left anterior descending, diagonal, rihght coronary artery, right coronary posterior descending lesions thus avoiding the deleterious effects of aortic crossclamp and pump oxygenator.
