HEMIRECTUS FLAP AND BILATERAL PECTORAL ADVANCEMENT IN MEDIASTINITIS

Today mediastinitis after coronary artery bypass surgery is a rare (< 1%) complication unless insulin dependent diabetes mellitus, bilateral internal thoracic artery use, older age and other major morbidities coexist. If mediastinitis occurs and can not be taken under control by means of classical treatment (partial sternal resection, debridement, Robiscek repair and closed mediastinal irrigation) in a low risk coronary artery bypass patient with left internal thoracic artery graft, best treatment might be right hemirectus flap with bilateral pectoral advancement reconstruction after complete sternal resection.
