Restrictive cardiomyopathy is a rare disease which has a poor prognosis if it is left untreated. Etiologically, it may result from secondary to some storage diseases and/or primary (idiopathic) or familial. The definitive treatment is heart transplantation. We herein report a case with familial restrictive cardiomyopathy which has been the only one case in our heart transplantation series with 27 cases since 1989. Its diagnosis, follow-up, familial screening, and its treatment by a successful orthotopic heart transplantation with literature reviews are presented.
