It is well documented that homozygous familial hypercholesterolemia might necessitate coronary bypass surgery in children. In this article, we present a nine-year-old girl admitted to our clinic due to respiratory distress and chest pain due to exertion which she had suffered for one month. The patient was suspected to have coronary artery disease and coronary angiography was done, revealing 90% narrowing in the left main coronary artery before the bifurcation point and osteal 90% narrowing in the right coronary artery. The patient underwent coronary artery bypass surgery and bilateral internal mammary arteries were used for the treatment of coronary artery disease.
