With the expanse and progress of the dialysis techniqucs, there is a significant risc in the number of the patients which have Chronic Renal Failure (CRF) and included in the haemodialysis programs in our country. Which procedure must be applied first to those patients awaiting renal transplantation, and also having cardiac problcms rcquiring open heart procedures is a matter of controversy. We decided in favour of mitral valve replacement in a patient who developed severe mitral insufficiency whilc awaiting renal transplantation. We present this case, who eventually died in the late postoperative period, with the techniques used during the operation and problems associated with CRF and haemodialysis in the postoperative period.
