Management in Tracheobronchial Iniuries

The records of 20 patients with tracheobronchial injuries from 1979 to 1993 were reviewed. Their ages ranged from 5 to 41, with a mean age of 19. Only one of them was female. the cause was gunshot wound injury in 11, traffic accident in 6, fall from height in 2, and animal bite in 1. In 11 patients cervical trachea, in 6 main stem bronchus, and in 3 lobar bronchus were injuried. There were concomittant esophageal injuries in 7 of the cervical tracheal injuries. Primary repair was done in 7 of the tracheal injuries and tracheostomy required in the other 4. 3 of the main stem bronchial injuries were repaired primarily. In the remaining two and in all patients with lober brochial injury died due to air embolism during the induction phase of anesthesia. One patient with tracheoesophageal injury died In the early hospitalization period and two patients with tracheoesophageal injuries died due to mediastinitis in the late postoperative period. We decided to report our experience In tracheobronchial injuries in the light of literature as they have high morbidity and mortality rates.
