Occlusion of all the aortic arch branches is called “Aortic Arch Syndrome” and it is a rare entity. Surgery is the choice of treatment for symptomatic cases. Transthoracic or extrathoracic surgical approaches can be used for surgery. In order to perform concomitant surgery, transthoracic approach must be the preferred tecnique for patients having coronary artery lesions along with aortic arch syndrome. This case report described a patient having both aortic arch syndrome and coronary artery disease. Transthoracic approach was used, and both pathologies were treated concomitantly and succesfully.
