One of the main factors which affects early and long-term mortality after heart transplantation is donor heart preservation. This step is one of the most important process during heart transplantation procedure and is directly related to normal or near-normal functions of posttransplant graft. Optimal protection of donor heart functions during ischemic period is only possible by means of adequate preservation method. This review presents physiologic principles of preservation, storage, reperfusion, heart preservation by perfusion, donor heart preservation, and clinical solutions, respectively.
