The incidence of anomalous coronary arteries is 1% in the population. Anomalous origin of the left main coronary artery arising from the right sinus valsalva is a very rare condition. The prognosis of the disease has not been defined clearly yet and currently there is no treatment algorithm available. This rare condition may present with exercise-induced angina, syncope and sudden death. However, as in our case, some of these patients may have long-term survival to older ages, due to the benign nature of the disease. In this article, we present a 75-years-old female patient with all coronary arteries arising from the right aortic sinus and advanced atherosclerotic stenosis of the left anterior descending artery, who underwent bypass surgery, discussing the surgical techniques which can be applied to such anomalies.
