The life expentancy is significantly decreased in patients with aortic aneurysm. It’s known that surgical therapy or another words, graft replacement therapy has prolonged the life expectancy of patients. More than one half of patients with aortic aneurysm had involvement of multiple aortic segments. In patients with multisegmental aortic aneurysm, persistent or recurrent aneurysm formation could be seen if only a part of aorta is surgically treated. Long-term studies indicate that persistent or recurrent aneurysm is a significant cause of death. Therefore the optimal surgical therapy of such patients in to replace all involved aortic segments in the same operation. However, the technique of total replacement should be short as to decrease the risk of irreversible tissue damage to the spinal cord and brain. Retrograde subtotal aortic replacement (Cooley operation) allows the entire aorta to be replaced with relative safety. We performed Cooley operation in 3 patients and discussed it with other treatment modalities in this study.
