Long standing pulmonary insufficiency can cause ventricular dysfunction. Two patients with severe pulmonary insufficiency, not responding to medical therapy underwent pulmonary valve replacement in our department. Both patients had undergone right ventricular outlow tract reconstruction in their youth. Both patients received biological valves. 12 months after the operation, radionuclear ventriculography showed an increased right ventricular ejection ratio and an improvement in functional class from NYHA class IV to II. Patients with severe pulmonary insufficiency and right ventricle failure not responding to medical therapy can undergo pulmonary valve replacement with a low risk and they seem to benefit from the operation in the short term.
