Complete Arterial Revascularization by Using Right Gastroepiploic Artery and Bilateral Mammaria Interna Artery Grafts

Two cases of coronary bypass surgery, in whom right gastroepiploic artery (IRGA) graft, was used along with bilateral mammaria interna artery grafts (SMA) were prensented. In the first case, ages 38, the left IMA was implanted to the left anterior descending artery, the right IMA was implanted to the right coronary artery and the RGA was implanted to the posterolateral branch of the circumflex artery. In the second case, aged 46, the left IMA was implanted to the left anterior descending artery, the right IMA was implanted to the first marginal branch of the circumflex artery, and the RGA was implanted to the right posterior descending artery. In both cases, all grafts were used in situ. Postoperative course of both cases were uneventful. The superiority in long-term patency of IMA grafts compared with saphenous vein grafts has been well documented. When the RGA grafts are considered to behave as IMA grafts, it would be understood why wee have utilized artery grafts in coronary artery revascularization.
