Intermittent veno-venous hemodiafiltration (IVVHD) is an alternative treatment method for renal functions especially in unstable patients.

IVVHD was applied to a patient with rheumatic valvular disease during the preoperative phase. The patient phase in congestive heart failure and the mean arterial blood preasure was around 50 mHg. The indication for IVVHD was volume overload as a result of oliguria. Vascular access was obtained through a double lumen venous catheter. In the femoral vein a roller pump applied between the venous line and the filtration membrane was used to provide ultrafiltration pressure.

IVVHD can be safely applied to hemodinamicly unstable patients with acut renal failure, during both the preoperative and postoperative phases. It has low complication rate and good at clearence of waste products.
