THROMBUS DISPLACEMENT DURING EMERGENCY CORONARY ANGIOGRAPHY IN A PATIENT WITH CORONARY THROMBUS LEADING TO ACUTE MYOCARDIAL INFARCTION

A 42 year-old man presented with an acute large anterior myocardial infarction underwent emergency coronary angiography with the aim of primary angioplasty. On coronary angiography a large thrombus leading to 95 % stenosis of the proximal left anterior descending coronary artery was detected. During the coronary imaging the thrombus broke out from the trunk spontaneously, and moved distally. The displacement of the thrombus was clearly seen on angiography. As soon as the thrombus reached to the distal segment of the vessel ventricular fibrillation occurred. The patient was resuscitated. For the possible remaining coronary thrombus and unstable plaque t-PA totally 100 mg / 60 minutes was infused. After five days control coronary angiography was performed, and none of the plaque formation and remnant of the thrombus were observed.
