A 50 year old woman was referred to us with dyspnea, tachycardia and limited effort capacity (NYHA CLASS III). Clinical examination revealed mitral insufficieny (MI) and cardiac failure. Her hematological and biochemical findings were normal. Chest X-ray displayed cardiomegaly. ECG shoed AF and VES. 2D-ECO Demonstrated MI 20, tricuspid 10, and PAP 40 mm Hg, and a ball-like mass connected to interatrialseptum in the left atrium, with pericardial effusion. There was 2 cam hypodense area in the left atrium with pericardial effusion, detected by thoraca CT. At the cardiac catheterization, coronary and pulmonary arteries were normal, however a mass and MI 20 were seen. Surgery was performed and two litters of straw-colored fluid recovered from the pericardial cavity and the mass was removed with its pedicle. Gross and microbiologic investigations showed pericarditis and myxoma.
The recent cardiac ECO disclosed MI 10, and a smaller heart. Her effort capacity was NYHA CLASS I due to medical therapy at the last check-up.
