Surgical treatment of a fast growing descending thoracic aortic aneurysm in an 80-year-old patient

Surgical therapy of descending thoracic aneurysms in advanced age has relatively higher mortality and morbidity. An 80-year-old patient underwent aortic valve replacement and ascending aortic graft interposition for the treatment of giant ascending aortic aneurysm and severe aortic insufficiency. He was reoperated for his descending aorta reaching a diameter of 80 mm in 18 months. His aortic valve was replaced under deep hypothermic circulatory arrest.The patient was discharged on the 15th post-operative day and remained asymptomatic during the 20 months follow-up.
