181 Patients with rheumatic mitral valve disease were performed open heart surgery in this study. Mean age was 36 years old. We performed 113 patient who underwent mitral valve replace ment, 28 patient who underwent open mitral comissurotomi, 17 patients who under underwent mitral valve replacement associated with aort valve replacement. All patients were especially stu died by using Transthoracic Color Doppler Echocardiografi (TTE) for left atrial thrombus. In addi tion to, left atrial thrombus were peroperativelly explored by surgon. Rare of detecting left atrial thrombus by TTE was found 63.6 percent and rare of false positive result was 36.4 percent and rare of false negative result was 47.6 percent. Atrial fibrillation was demonstrated 90 percent of patients with left atrial thrombus.

As the result of; especially Rheumatic Mitral valve Disease patients with atrial fibrillation must be decided to make operation with not only TTE but all clinical findings.
