Right anterior submammarian minithoracotomy approach in "less invasive" aortic valvular procedures, which we believe is superior cosmetically especially in female patients, is
presented. This approach was applied to 8 patients among which 5 had aortic stenosis and 3 had subaortic stenosis. Lenght of the incisions were 7 to 12 cm, and the thoracotomies were performed at the 3rd intercostal space by subpectoral approach. Conventional surgical equipment were used throughout the procedures and all cannulations were done through the thoracotomies. Mitral valvular procedures and some congenital cardiac operations can be conducted through the same incision but by entering the thorax through the 4th intercostal space.
