Surgical Management of Peripheral Lung Tumors Adherant to The Chest Wall

From 1988 through 1992,19 patients underwent operation for non small-cell carcinoma of the lung invading the chest wall. All patients were male with an average age of 56 years.

Chest pain was an initial symptom in 52.6%. Enbloc resection of the chest wall and lung was performed in 12 patients, partial resection in 4 patients and extrapleural resection in 3 pati ents. The median number of the ribs resected was two (range: 1-5) and only one patient re quired chest wall reconstruction. Pathologic staging was T3N0M0 in 47.3%, T3N1-2M0 in 31.5% and T4N0M0 in 21%. Radiation therapy was employed in 10 patients (52.6%) for posi tive mediastinal lymph nodes or positive resection margin. Patients who had incomplete re section (positive resection margin) had a one year median survival. 11 of 12 patients under went curative resection at follow up are disease free (range 9 to 59 months).
