Bilateral coronary artery fistulas are rare congenital malformations. A 29-year old male patient with this rare anomaly is presented. The coronary angiography showed two fistulas; one of them originating from the proximal right coronary artery and the other from the left main coronary artery with both of them draining into the proximal main pulmonary artery. The fistuluous tracts on both sides were identified, dissected and ligated proximally, multiple vascular structures on the pulmonary artery wall were sewn up.
