Azygos lobe is a rare congenital anatomical variation seen in 0.1% to 8% in the general population. Its association with bullous changes is even rarer. A 40-year-old male patient presented with a complaint of shortness of breath. Radiological investigations including plain chest radiography, chest computed tomography, and lung perfusion scintigraphy showed bullous lesions in the right upper zone. Following a right thoracotomy, an azygos lobe was detected in the right upper lobe, with 75% having a bullous structure. Azygos lobe was resected. There were no postoperative complications.
