Hybrid surgical approach to graft thrombosis

In this article, we present our experiences on peripheral
vascular surgery following pharmacomechanical
thrombectomy in a case of thrombosis in his leg, which was
performed aortobifemoral bypass graft. Forty-nine-year-old
male patient had a necrotic wound at his left foot little
(5th) finger. Angiography revealed a blockage in the left
leg of aortobifemoral bypass graft. When conventional
embolectomy procedure under the spinal anesthesia
failed, thrombectomy was performed with a Cleaner
thrombectomy catheter. Graft was anastomosed to arteria
profunda femoris and the saphenous vein harvested from
the ipsilateral lower extremity was interpositioned between
the graft and infragenicular popliteal artery. We believe
that the hybrid surgical interventions in combination with
pharmacomechanical thrombectomy are useful treatment
of choices to save the graft and extremity in cases in whom
conventional thrombectomy is inadequate.
