Dumbbell tumors originated from intradural nerve sheath are approximately 10% Posterior mediastinal tumors. These tumors are defined as dumbbell or hour-glass tumor and are composed of a great intrathroracic and a smaller intraspinal components connected through intervertebral foramen. Preoperative evaluation included magnetic resonance imaging (MRI) and / or / myelogram / computed tomography (CT). MRI is sensitive than magnetic resonans imaging for identification of intradural tumor extention. Surgical management of hour-glass tumors bay a team including neurosurgeons and thoracic surgeons decreases serious complications such as hemorrhage, spinal cord injury, or cerebral spinal fluid leak. In this report we present three cases and describe diagnosis and surgical approach.
