Surgical correction in truncus arteriosus type IV is one of the most diverse and demanding operations ile congenital cardiac surgery. Particularly creation of the pulmonary artery-right ventricle continuity and the closure of various aortico-pulmonary collaterals necessitates a different approach to each patients. Here we have presented a case in whom a bioprosthetic valved conduit was used for formation of the right ventricle outflow and who underwent two reoperations for closure of aortico-pulmonary collaterals undiagnosed during initial evaluation. With this presentation we have reviewed surgical correction principles and pitfalls in correction of this rare and frustrating malformation.
