Thymic cysts of the mediastinum are uncommon, accounting for only 1% to 3% of all mediastinal masses. They may be congenital or acquired. A left anterior mediastinal mass was incidentally detected on a chest radiograph of a 40-year-old asymptomatic man. Computed tomography showed a cystic mass, 5x6 cm in size. The cyst was totally resected via left thoracotomy. Histopathologic diagnosis was made as unilocular thymic cyst. As no etiologic factor was determined, it was considered a congenital cyst with atypical features. It had a considerably thick cystic wall with calcifications; in contrast to congenital unilocular thymic cysts, the contents of the cyst were not soft in consistency.
