Aim of investigation: Despite substantial progress in recent years, acute type A dissection involving the ascending aorta remains a serious condition.
Methods: From September 1994 to October 1995, 5 patient (3 men, 2 women) underwent surgical repair a type A acute dissection involving the ascending aorta at Bursa Yüksek İhtisas Hospital. Subject ranged in age from 31 to 69 years (a mean age of 55 years). The major symptom of all patient was severe chest pain. Diagnosis is done by chest X-ray, transesophageal echocardiography, and cardiac catheterization. Distal anastomosis was performed with deep circulatuar arrest in 4 patient. Retrograde cerebral perfusion was performed 3 patient. Myo- card was preserved by antegrade and retrograde cor- onary sinüs perfusion. Ascending aorta replacement vvas performed by a Dacron conduit in all patient. Aort Valve Replacement in l, and Coronary Bypass grafting in 2 patient was done.
Result: Mean total circulatuar arrest time was 20 minutes. Early mortality was l (% 20) due to renal failure. Mean follow-up was 11 months (2-16 months). There was no late mortality and mobidity.
Conclusion: We belive that early diagnosis and adequate management of acute type A involving the ascending aorta have led to a decrease in the mortality and major increase in long-term survival.
