Extra anatomic bypass is the alternative surgical method in case of graft occlusion or infection when abdominal approach is contrindicated for aorto-iliac reconstructions. Last two decades, axillo-femoral bypass was popularized and was frequently performed. However, high mortality and morbidity rates, low long term patency rates and necessity for reoperation made the surgeons look for alternative procedures. Thoraco-femoral bypass presents optimal inflow source, satisfactory patency rates as conventional aorto-femoral bypass and a superior alternative to axillo-femoral bypass. Extra-anatomic bypass with thoracic aorto as an inflow source demonstrates a different way of surgical approach for the indications, operative strategies and hemodynamic benefits.
