Open Heart Surgery in Patients With Chronic Hemodialysis Dependent Renal Insufficiency

By the application of dialysis techniques, the quality of life and long-term prognosis are improved for end stage renal failure at the last years. Hemodialysis patients suffer a higher risk of morbidity and mortality secondary to cardiovascuiar disease. The mortality rate is 30-53%. We performed a retrospective analysis of 16 patients with end stage renal failure who undenvent extracorporeal circulation and beating heart by-pass surgery during Jan. 1995 and Oct. 1998. Four of 16 patients were women. The functional classification stages of the patients were: Stage II (4), Stage III (8), Stage IV (4). The conditions leading to open heart surgery were: Coronary artery by-pass surgery (11), valve replacement (3), coronary artery by-pass surgery and valve replacement (1), VSD restoration (1). The early mortality is l (%6.2). The average length of stay in the intensive care unit is 3.4 days (1.5-26 days). The average hospital stay İs 11 days (7-26 days). Patients on chronic dialysis should undergo cardiac surgery if the preoperative diagnostic steps confirm the indication and the operations can be performed with accetable morbidity and mortality.
