The aneurysms of saphenous vein grafts to coronary arteries are a rare complication of coronary artery bypass grafting (CABG) surgery.

A 68-year-old male who had undergone CABG 5 years ago, presented with dyspnea and angina pectoris. Cardiac catheterization and echocardiography demonstrated a 3x4 cm saccular aneurysmal dilatation of the ascending aorta and three vessel coronary artery disease.

At the time of operation, aortocoronary saphenous vein (grafted to the left anterior descending artery) was found aneurysmatic at proximal anastomosis site. The aneurysm was succesfully resected and the patient underwent triple CABG.
