Medtronic-Hall ve St. Jude Medical Kapak Replasmanı Olgularının Erken Postoperatif Dönemde Hemodinamik Açıdan Karşılaştırılması

Early postoperative haemodynamic findings of the 75 Medtronic-hall (MH) valve replacement cases, and 165 St. Jude Medical (SJ) valve replacement cases were evaluated retrospectively to compare the haemodynamic performance of these valves. Regressions of the pulmonary hypertension, diuretic and inotropic drug requirements and stroke volume index (STRVI) parameters were compared in both SJ and MH group of patients treated with mitral valve replacement, aortic valve replacement and double valve replacement.

Early postoperatie STRVI was 46.3±12.1 and 43.1±13.2 ml/m2 in SJ and MH groups respectively. In the early postoperative period, 83 patients (%50) in SJ and 40 patients (%53) in MH group required inotropic agent (dobutamine) infusion. Average diuretic (furosemide) requirements were 77±35 and 67±43 mg per patient in intensive care in SJ and MH group respectively. Pulmonary artery mean pressure decreased from 34±12 to 17±8 (p<0.001) in SJ and also decreased from 32±13 to 17±6 mmHg (p<0.001) in MH group. The hospital mortality was %3 and %2.6 in SJ and MH groups respectively. The differences in the parameters compared between two groups were not found statistically significant and both prosthetic valves were thought to be positive and reliable in providing good haemodynamic performance in the early postoperative period.
