Chronic atrial fibrillation (CAF) is a common arrhythmia. CAF is prevalent in 0.4% of the general population. Symptoms and ventricular dysfunction may be caused by a rapid ventricular response to CAF. But pharmacologic control of rapid ventricular response to CAF can be difficut. For this reason alternative transcatheter approaches have been developed to control ventricular response in durg-refractory cases including radiofrequency, direct current energy or chemical AV node ablation modification. It seems appropriate to attempt. AV node modification prior to AV node ablation in patients with drug refractory CAF and rapid ventricular response, in order to avoid the need for permanent pacemaker implantation.
