Emergent Surgical Revascularisation in Acute Myocardial Infarction

Early reperfusion is cornerstone of therapy in evolving myocardial infarction (AMI). Emergent coronary bypass (CABG) indicates for patiens who are failed trombolytic therapy and conterindicated angioplasty (PTCA) and suffered from PTCA failure.

18 patients were operated due to AMI. Spontaneous evolving AMI has occurred for nine patients. Another nine patients have been experienced AMI due to PTCA failure.Two patients (11, 11%) were admitted operating room with cardiogenic shock and one patient (5.55%) with cardiopulmonary resuscitation. Mean graft number for per patient was 1.61. Aortic cross-clemping and cardiopulmonary bypass times were 23.1 min. And 83.02 min, respectively. Ischemic interval was 118, 46 min. Postoperative mediastinal bleeding was 664.61 cc. Time to respiratory support and period of intensive care unit were 10.23 hours and 3.31 days, respectively. 15 patients (83,33%) were reperfussed successfully according to electrocardiography. Superficial sternal and deep sephen wound infection and postoperative reversibl neurologic deficit developed in three distinct patients. Two patient (11, 11%) died.

Emergent CABG could be performed with relatively low mortality and morbidity for the treatment of AMI that is a life-threatening situation.
