Deep Hypothermic Total Circulatory Arrest Application During Aortic Surgery

Depp hypothermic total circulatory arrest firstly introduced by Griepp for aortic surgeryin 1975.With this improvement surgical treatment o some complex cases become possible. After clinical application of circulatory arrest and open repair technique, deep hypothermic total circulatory arrest has found a great place in the treatment of acute aortic dissections. During January 1990-june 1992,10 cases have been operated with depp hyothermic total circulatory arrest for aortic dissection in Cardiovascular Surgery Deparment of Ege University Medical Faculty. Dissectionc were acute in 8 cases and chronic in 2 cases. Three patients were lost, 7were discharged from the hospital without any problems.

Slow cooling and rewarming, topical cooling, alpha-stat principle for blood gas magement, avoidance of hypoglycemia and hyperglycemia, hemodilution and pharmacologic protection are basic principles of total circulatory arrest application.

Even with optimal management we have to know that arrest period more than 45 ninutes may be hazardousk for the patients.
