Cardiak transplantation has taken its place as an important therapeutic intervention for the end-stage heart disease. One year survival rate has increased to % 80 . Infection and organ rejection are still the major problems. Endomyocardial biopsy (EMB) which is current. It is crucial to differentiate the acute rejection pathology from Quilty phenomenon that appears in patients under cyclosporine therapy. We have seen this entity in one our patients at the 4th EMB after cardiac transplantation.
