Ebstein Anomalisinin Tanı ve Tedavisinde Ekokardiyografinin Yeri

Ebstein’s anomaly, involving the tricuspid valve and right ventricular myocardium, is a congenital disease. As the echocardiographic diagnosis of this anomaly can be made very accarately, angiography is no longer necessary in the majority of cases. Echocardiography is used for recognization of the morphological features of anomaly, functional assessment, plannign of type and timing of surgical intervention and follow up of the patients. Echocardiography was used for this purposes in our study. 4 patients (two women, two men) were investigated with regard to the diagnosis of Ebstein’s anomaly and the essential echocardiographic criterions in the indications of treatmeant. The common morphological features of tricuspid valve in 4 patients were displacemon pathology was tethering (chordal attachments of the tricuspid valve to the underlying myocardium) and dysplasia when each leaflet investigated separetely. The displacement index was more than 8 mm/m2 in all patients. The anomaly most frequently associated with Ebstein’s anomaly was mitral valve prolapse (MVP). The patients were evaluated according to the echocardiographic anatomical and functional severity of Ebstein’s anomaly. The most common criterions of right atrium and ventircule. Severe tricuspid regurgitation in 4 patients, right ventricular dysfunction in 3 patients, atrial septal defect (ASD) with right-to-left shunting 2 patients was shown in the functional severity assesment of the patients. Valve repair in one patient, valve replacement in other 3 patients was planned with the assistance of echocardiographic findings.
