Modified Blalock Taussig shunt operation is used frequently in palliative management of cyanotic heart diseases. Perigraft seroma may develop after modified Blalock Taussig shunt operation due to politetrafluoroethylen grafts. Spontaneous regression may occur in most of the cases. It is very unusual that this complication causes life-threating symptoms. We report here a case of giant perigraft seroma presenting with mediastinal compression symptoms and its surgical treatment. Thereby, we would like to emphasise that perigraft seroma may be life-threating in rare occasions and close follow-up is necessary especially in patients with recurrent serous drainage after the operation.
