Early surgical intervention has a major significance for penetrating cardiac injuries to the heart. A patient with a stab wound on the heart and abdomen was urgently operated in another clinic. In a few days cardiac failure developed and a ventricular septal defect was suspected. Therefore he was transferred to our clinic for detailed evaluation and surgical treatment. The patients was immediately operated after verification of the diagnosis and the defect was repaired with success. After lifesaving urgent operation for penetrating injuries to the heart the possibility of the existence of an intracardiac lesion has to be considered.
