The frequency of upper extremity peripheral artery aneurysms is much less than lower extremity peripheral artery aneurysms. Their diagnosis and surgical treatment is important because they can cause functional loss such as upper extremity and finger losses. In this study we investigated the diagnosis methods and surgical therapy of a case which was completed its maturation 5 years after a gunshot injury and admitted to our clinic at 7th year under the light of literature knowledge. Surgical therapy of brachial artery aneurysm is critical because it may cause thromboembolism in that extremity which may end up with gangrene or amputation. Besides the first choice of immediate surgery and routine surgical repair, we recommend selective revascularization procedure.
