Simultaneous spontaneous bilateral pneumothoraces are rather rare cases in the world literature. The patient has applied to our emergency department with the complaint of the respiratuar distress and chest pain. Chest rontgenogram confirmed total pneumothorax on the left and partial pneumothorax on the right side. Simultaneous tube thoracostomies were carried out both the left thorax and the right thorax. As air leak stopped on the right thorax and the right lung reexpanded on the chest rontgenogram in the fourth day, the thoracic tube on the right was removed. Left mini axiller thoracotomy was carried out because of the persistent air leak and expansion default on the left thorax. Partial pneumothorax on the right thorax recurred on the second postoperative day. The patient was operated through right mini axiller thoracotomy incision Subpleural blebs were seen in the apical segment and apicoposterior segment and bleb resectins and pleural abrasions were performed in both sides. Thorax drains were removed on the second and fourth consecutive days postoperatively. The patient was discharged from the hospital on the fifth postoperative day. The treatment and results are discussed and the recent literature is rewieved.
