Management of critical pulmonary stenosis in young
infants includes surgical reconstruction of the right ventricular
outflow tract, closed pulmonary valvulotomy,
and transcatheter balloon valvuloplasty. A 14-day-old
infant with pulmonary stenosis was treated with closed
pulmonary valvulotomy with the inflow occlusion procedure.
The transvalvular pressure gradient which was
148 mmHg preoperatively decreased to 45 mmHg at the
end of one year. This technique is safe and effective in
selected cases.
