Aortocoronary bypass was performed in a dialysis-dependent patient with chronic renal failure. He had symptoms of ischemic heart disease for seven years and a recent myocardial infarction. Angiocardiography revealed a triple vessel disease. Hemodialysis performed one day before the operation provided optimal conditions. Triple aortocoronary bypass was undertaken with saphenous vein grafts. For the first four postoperative days fluid-elcctrolyte balance and acidosis were maintained by hemodiafiltration. There were no bleeding problems in the por and postoperative periods. Despite the increased morbidity and mortality risks in dialysis-dependent patients, coronary bypass operations should be considered to improve survival and life quality.
