Bronchopleural fistula after lobectomy is a rare complication and has a significant mortality. Management options include systemic antibiotics, adequate pleural drainage, removal of necrotic tissues and obliteration of residual pleural space. In this case report, the place of transsternal-transpericardial approach, especially in the management of bronchopleural fistulas associated with bleeding from the previous bilobectomy pulmonary arterial stump was evaluated.
