CAN EPICARDIAL ELECTROCARDIOGRAM BE USED IN DIAGNOSIS OF REJECTION AFTER HEART TRANSPLANTATION DURING EARLY PERIOD?

A successful orthotopic cardiac transplantation was performed with classic technique in a twentythree years-old female patient who had familial restrictive cardiomyopathy and internal cardiac defibrilator. Postoperatively electrocardiographic recordings were obtained from the epicardial pacemaker lead. On the postoperative 13th day, rejection had been detected clinically and echocardiographicaly. In the electrocardiographic recordings at the same day a decrease of R wave amplitude was observed. Endomyocardial biopsy was not performed. Clinical and echocardiographic recovery was achieved with a three days steroid therapy and R wave gained the initial amplitude.
