Surgical Repair in Ruptured Congenital Sinus of Valsalva Aneurysms

Rupture of a congenital aneurysm of the sinüs of
valsalva is a rare congenital cardiac malformation.
Between 1985, and June 1997 at the Koşuyolu Heart
and Research Hospital, we have operated 28 patients
with ruptured sinus valsalva aneurysm. This
represents 0.26 % of all open cardiac operations
(28/10700). The major cause of them (67.8 %) found
to be rupture of a congenital aortic sinus aneurysm,
usually during the third or fourth decade of live. The
origin of ruptured congenital sinüs valsalva
aneurysm was the right coronary sinüs in 17 and the
noncoronary sinüs in two cases. We have not seen
any ruptured sinüs valsalva aneurysm which
originated from left coronary sinüs. The aneurysms
ruptured into the right ventricle in 13 patients, into
the right atrium in five patients, and into left
ventricle one patient. Venticular septal defect was
the most common associated defect (6/19, 31.6 %).
The patient ages ranged between 14 and 55 years
(mean age 28.5). For the repair of ruptured sinüs
valsalva aneurysm vve have opened aorta and the
cavity into vvhich the aneurysm has ruptured. The
aneurysmal sac was excised and the defect was
closed with a patch. The hospital mortality rate was
5.2 % with one patient. Operative management of
patients with ruptured congenital sinüs valsalva
aneurysm is discussed.
