Two patients without prior vascular disease developed unilateral right dropped foot and compartment syndrome with myonecrosis within 12 hours following uncomplicated coronary surgery. Risk factors have been searched. The common thread in each case was a tight elastic wrap that was applied before cardiopulmonary bypass which was associated with a low systolic blood pressure (low cardiac output). Especially on patients with peripheral vascular insufficiency keeping leg in comfortable position and avoiding the elastic wrap until after CPB will eliminate this complication.
