We performed a succesfull mitral valve replacement operation in a patient with serious mitral valve regurgitation and chronic idiopathic trombocytopenic purpura. Preoperative platelet count was 38.000/ml and the patient was treated with high dose intravenous immune globulin. After an increase in platelet count, operation was done with a short cardiopulmonary bypass time. There was not any perioperative platelet
transfusion. Simultaneous splenectomy was not done.
